Reissuance of Revoked Certificate of Registration, Replacement of Lost or Damaged Certificate of Registration, Professional Identification Card or Temporary/ Special Permit. The Board may, upon petition on meritorious grounds, reinstate or reissue a revoked certificate of registration after two (2) years from the date of effectivity of the period for revocation. The date of effectivity of the period of revocation shall be reckoned from the date of receipt by the respondent of the final order or resolution revoking his/her registration or the date of the surrender by the respondent of his/her certificate of registration and professional identification card, if it is still valid.

The Board may not require the holder thereof to take another licensure examination. The petitioner shall prove to the Board that the ground or grounds for the revocation of his/her certificate of registration no longer exists. For the grant of his/her petition, the Board shall issue a Board Resolution subject to approval by the Commission.

A duplicate copy of lost certificate of registration, professional identification card or temporary/special permit may be reissued in accordance with rules thereon and upon payment of the prescribed fee therefor.
