Indication of Numbers: Certificate of Registration and Professional Identification Card, Professional Tax Receipt (PTR), AIPO Membership and Official Receipt (O.R.). - The nutritionist-dietitian shall be required to indicate his/her certificate of registration number and date of issuance and the expiry of his/her current professional identification card, PTR number, AIPO membership number and O.R. number on the documents he/she signs, rises, or issues in connection with the practice of nutrition and dietetics.
