Reinstatement, Reissuance or Replacement of Certificate of Registration and Professional License. - Two (2) years after the date of revocation, the Board may, upon application and for reasons deemed proper and sufficient, reinstate any revoked certificate of registration and reissue a professional license, and in so doing, may, in its discretion, exempt the applicant from taking another examination.
A new certificate of registration or professional license or special permit, to replace lost, destroyed or mutilated ones may be issued subject to the rules as implemented by the Board.
