IEP Preparation and Review. -With the active participation of the special needs teachers, in collaboration with the multidisciplinary team and with the consent of the parents or guardian of the learners with disabilities and the learners with disabilities themselves, when appropriate, IEPs for all learners with disabilities who require specially designed instruction and educational support services and programs shall be prepared by the schools, the CDCs, and the ILRCs.
The IEPs shall be based on the educational assessment and diagnosis of the learners with disabilities by the multidisciplinary team and other relevant specialists. The parents or guardians of the learners with disabilities shall be given a copy of the IEP.
The IEP shall likewise be subject to an annual review or at such other period as may be determined by the multidisciplinary team, which shall not be later than one (1) year from its implementation to assess the progress of learners with disabilities, and revise the plan when necessary.
An IEP review shall likewise be conducted upon the request of the parents or the school for its early review if there is a determination that such review is necessary. The result of the IEP review or the necessary of revising the same shall be communicated with the parents or guardian of the learners with disabilities and the learners with disabilities themselves, when appropriate.
