Reissuance of Revoked Certificate of Registration and Replacement of Lost Certificate of Registration and Professional Identification Card. - The Board after two (2) years from the date of revocation of the Ceritificate of Registration may reissue a certificate upon proper application.
A new Certificate of Registration and Professional Identification Card or temporary/special permit, which has been lost, destroyed or mutilated, may be reissued after payment of the required fee prescribed by the Commission.
