Reissuance of Revoked and Replacement of Lost Certificate. – The Board may issue for reasons of equity and justice, upon proper application and submission of an affidavit of loss, another copy of the certificate of registration upon payment of the fee prescribed by the Professional Regulation Commission.
A new certificate of registration to replace any certificate lost, destroyed, or mutilated may be issued subject to the rules of the Board upon payment of Fifty pesos (P50.00).
